Welcome to the world of FertilityCare Creighton Model fertility awareness. Some know it as Natural Family Planning, or NFP. You have questions and concerns. You may have been thinking of them for a while. Your doctor may have directed you to learn to chart your fertility signs daily with the Creighton Model.
Call your insurance to make sure diagnosis code Z30.02 is covered under Tepeyac. Z30.02 should read “instruction in natural family planning”. Ask how many sessions are covered, and the amount of your copay, if any.
Please make sure you are fully registered as a Tepeyac patient. To do this, call 703.273.9400. Make sure your current insurance information is on file.
After you have completed the above, email me at metroDCfertilitycareRN@gmail.com to arrange an appointment and delivery of paper materials you’ll use as a daily note taking chart. Include your patient number, your date of birth, your name on file with Tepeyac, and why you are seeking info for the Creighton Model.
